他脑袋正常吗? Is he all there? 不是指“他现在在哪里”。句中的all there 是指“脑袋处于正常的状况下”。所以这句话其实是用来质疑对方的脑袋是否正常，比如当你不相信对方干了什么傻事的时候： A: My brother just got arrested for robbing a store.
